Once i have created a custom form what is the easiest way to access and input

To use your customized form, you'll want to turn on the Import Style feature first.

Here's how:

1. Click on the Gear icon.
2. Under Your Company, select QuickBooks Labs.
3. Look for Import Style, and toggle the switch to on.
4. Click Done.

Then, here's how you can import the customized form:

1. Click on the Gear icon.
2. Under Your Company, Click Custom Form Styles.
3. Click on the New Style drop-down, and select Import Style.
4. Click Browse and select the template file.
5. Click Next to upload your document, then map the fields.

Is there any way of choosing the form using the app, or is the default the only option?

I wanted to create an estimate form that doesn't have totals - I did this, but it isn't adding tax to each line item..  How can I do this?  I want each line item to have its own amount + tax, so that customers can decide which option they want to go with.

QuickBooks Online (QBO) mobile app lets you access your business data instantly. However, selecting a form style or a template form from your mobile is currently not available. You’ll want to log in to your account via a regular web browser to create and choose a form style from there. I’d be glad to guide you how:

1. Go to the Gear icon.
2. Select Custom form styles.
3. Click the New style drop-down and select Estimate.
4. Go to the Content tab.
5. Select the Table section.
6. Uncheck the box Rate and Amount then click Done. 

To apply the form style, create or open an estimate. Click the Customize option from the bottom of the estimate and select the template you’ve created.

In regards to adding a tax for each line item, I suggest looking for a third-party app that has this option. You can go to the Apps menu from your QBO account and browse an app from there.
