I appreciate what you've done so far, @mattstas1234. Let's perform troubleshooting steps to ensure you can add a date in the default date column in your invoices.
To begin with, please ensure to turn on the service date feature in the accounts and settings.
Here's how:
If the same thing happens, there were times when local internet cache files stored in the system could cause unexpected behavior in the product. This is why the service date column never shows the date column and you can't add one on your end. Let's fix this by accessing your QuickBooks account in a private window. You can use either of the following keys:
Once you can already add a date, you can go back to the regular browser and clear its cache to speed up the process when creating an invoice in QuickBooks Online. If you get the same result, I recommend switching to a different supported browser.
Additionally, you can check this article if you want to personalize your sales forms: Customize invoices, estimates, and sales receipts in QuickBooks Online.

Thanks for your input but it did not fix my problem.
I believe I have fixed it. When creating an invoice I went to "manage" and under "customization" the service date toggle was off. I toggled it on, entered a service date on the invoice, then saved the invoice. I then went back and toggled off that setting and saved the invoice again. Now the blank date column does not appear to continue to show itself.
