First, let's make sure to add correct hired date of your employee's. Here's how:
Also, make sure to uncheck the the Have you filed a new-hire report with the state? checkbox. Here's how:
If everything is properly setup, we can open your company in a private window or incognito mode. You can use any of these keyboard shortcuts to access one:
If everything works fine, go back to the regular browser and clear its cache. Also, you can use another supported browser if you get the same result.

The employer has no way knowing that the new hire has checked this box. So this means the employer is unaware that QB has failed to file the new hire report reporting with the state.
Please fix this and add a confirmation report so that we can see when and if QB has filed the new report with the state.
