I'm having the exact same problem. All these verification codes each time, but it doesn't actually download transactions. Frustrating that Intuit isn't working to correct this issue. For now, I just have to manually input transactions.
I had problems connecting QBO to LiveOak too. I spoke with a representative at LiveOak and she informed that in order to connect with QBO your LiveOak username must be 7 characters or less and all lowercase. Also, your password has to be 11 characters or less. I made the changes and I was able to connect. (At least the first time). It would be good for that information to be posted somewhere by LiveOak or Intuit.

I tried the lower case user name and short password.
The issue is that QB is trying to link to https://www.liveoakbank.com/ but that is not the correct URL because once you get to that URL you must click on Sign in, then select your banking style. The issue is that the URL needs to be https://secure.liveoak.bank/login. QBO needs to link to that URL not the previous one. @support should figure this out.

When you connect your bank to online banking, you don't have to open your bank URL but paste it into the field. Once you click your bank you'll be prompted to log in with your bank credentials to complete the connection. If you're being prompted to enter your Access ID and PIN, then it's possible that your bank requires additional verification. See the screenshot below for your reference.
If you don't have these credentials or haven't set up an Access ID and PIN, click the link from the highlighted message. Then, you'll be prompted to sign in with your bank's username and password.

This still does not work as a solution. I'm having the same issue, talked with your support & technical team, had a ticket open, and there is STILL no resolution. It's been 2 years now since the original post of the issue. There HAS to be something done differently by now.

Their research has validated that this is a valid issue with login credentials. If you’re still seeing this error, please log in to the bank/card issuer website outside of QuickBooks to ensure your credentials are accurate, and then use those same credentials in QuickBooks to access your account. Our team also recommends using the Show Password option whenever connecting your accounts to ensure the best possible experience.

@vdor I have long since moved on from LiveOak because of this very issue. If they can't solve the connectivity problems with QBO for their Business Savings customers, then they don't care enough about my business to keep it. So I took my 6-digit balance and moved it to a new bank that does keep its connections with QBO.
LiveOak just got a new CEO, so maybe this will change in the future. Too late for me, though.
Great point. I'm a new customer with them (6 months or so)...so if it doesn't change soon, I'll be moving on as well.
Where did you end up going? Alexis F. from LiveOak was rude, inconsiderate, and a plain old fashioned jerk telling me it was the username, the password, and I tried and changed both to still have the same results. Tired of having xx,xxx tied up with them when I can't even do proper accounting without a three ring circus.

I now use Wealthfront's Cash account, which provides better APY anyway. And it stays connected in QBO like a modern web app.
