how do I know if OR PFML is set up correctly in quickbooks online, is there a way to check?

I have set this up according to these instructions, but Quickbooks is still not withholding the tax for Paid Leave. It shows as "0" in my form OQ that Quickbooks generated. I am now receiving "Failure to File" notices from the state of Oregon.  

I am very disappointed in Quickbooks Payroll, I signed up so it would handle taxes on my behalf, but it does not.
