All the sudden the 3rd address line (city state and zip) disappeared from our printed pay stub (to screen and printer). We don't know how to get it back. Any suggestions?

I've got some troubleshooting steps we can perform to show the city, state, and zip address on the pay stub, mathpdaes.
Let's make sure were enter the address on the Contact Information page. This will show when printing the pay stub.
Here's how:
Then, we can go to the Paystub Printing Preferences to ensure the Company address is enabled. Let me show you how:
Then, try printing the paystub again.
If the same thing happens, it's possible there's a damaged data in your software. We can run the Verify and Rebuild Data utilities to fix any data issues. Just follow the steps and details in this article: Verify and Rebuild Data in QuickBooks Desktop.
