Thank you for your response. This problem started after a recent payroll update.
First of all in the form selection menu, instead of NYS45 having a Quarterly status, it has ANY.
Second, once the form with the correct dates and quarter is selected, it fails to populate schedule C with the employees data and other relevant details and totals. This results in an ongoing error.
I have reached out to QB Payroll Customer support and was told that they are aware of this problem with the update and their engineers are working to fix it with a new update to correct this problem.

Same issue. When will there be a fix?
Same problem - just went to file the 4Q NYS-45 and NYS-45-ATT - Quarterly Combined Withholding and it is not loading the NYS 45 ATT attachment which is what is causing the error to show that the totals don't correspond. As someone else mentioned, in the Payroll Center > File Forms tabs where you select these forms under Filing Period column it is not listed as a "Quarterly" form anymore, now says "Any" and you have to select the start and end dates. Don't know if that's part of the issue. But NYS-45-ATT is not showing up and I even downloaded and printed it out and the ATT was not part of it. I also just did the payroll update right before trying to completed this task. I restarted Quickbooks twice and tried Payroll Update twice just to be sure and it did not change the issue.

There is a problem with Form NYS-45 (Page #2, Part C) that prevents e-filing. I spoke to QBS about week ago and they were supposed to fix the problem. There is massage about Payroll Update 22504 to be released today 01/23/2025. I hope it will address the issue. Due date to submit 4QT2024 form is within just few days. QBS keeps increasing their prices (almost 300% every year!) and they are terrible with any support.
I'm having the same issue. It won't even let me type the employee info or override.

Hi QuickBooks, the problem is on your side and you know it. Please just fix the issue.
This is over a week and this problem has not been resolved. My subscription has tripled and this is the quality of service? We have deadlines to meet, and I just checked and there are updates with fix. The NYS45 still indicates filing period as " ANY" and not "Quarterly". Form still has error and does not populate Schedule C.
For the astronomical increase in the new absurd pricing, I would expect better customer support and quick fixes. I keep getting the same robotic answers from customer support - "engineers are working on this problem, we are unable to tell you when it will be resolved".
