Since re-opening your merchant account requires sensitive information, I'd suggest getting in touch with our Merchant Services Center to help you renew the account. All accounts related concerns need to be directed to our phone support for security purposes.
Once re-opened, you can go through this article to help you set up your invoices: Create invoices in QuickBooks Self-Employed. On the same link, you'll find steps on how to customize your template as well as managing your transactions.
Additionally, I recommend editing, assigning, and categorizing your transactions. Doing so will help you ensure the accuracy of your financial records.
