Thanks for using the 1099 E-File Service in QuickBooks Online (QBO).
The 1096 forms isn't required by the IRS when you e-file the 1099 forms. That is one less piece of paper. Check out this link for the 1099 FAQS: File 1099 Online.
With regards to your second question, Massachusetts is one of the states that support the Combined Federal and State Filing (CF/SF) Program. This means that IRS will forward the electronically filed 1099 forms to Massachusetts. Therefore, separate reporting to your state is no longer required.

I ran into the following questions
1. vendor has foreign address and QBO shows error message, i cannot enter foreign address

You'll want to get a form W-8BEN from IRS and signed by your foreign vendor. Signing the form W-8BEN certified that the foreign vendor is not a U.S. citizen.
Thus, form 1099 and withholding are no longer required for a foreign vendor.
You can get the form W-8BEN from the IRS.

If you need to file, you can also ask which address you should use on the form, especially for those freelancers that don't have a permanent address abroad. However, please keep in mind that QBO only supports the US address format. So you may look for other options for filing in that case.

In QuickBooks Online (QBO), being able to file the Combined Federal State Filing is currently unavailable. However, you can use the services in QBO to file it electronically which is the 1099 E-file services. Using this feature, you can file your 1099-MISC and 1099-NEC filings.
