Was not able to send you a screenshot afterall as the "filter" deleted the file at intuit.com.
The MTD VAT screen after uploading the csv spreadsheet showed a message: "Something went wrong. Try uploading your spreadsheet again or come later if you still have issues."

However, if the issue persists, I'd suggest performing some browser troubleshooting steps. First, let's try accessing your QBO account on the incognito window. This way, we can check if it's a browser-related issue. Here are the keyboard shortcuts:
Once done, upload your spreadsheet again. If it works, we have to clear your browser's cache to optimize your browsing experience. You can also try using a different browser. Here's the link: System requirements for QuickBooks Online.
I'd like to include a few articles about importing data in QuickBooks Online. We'll surely get additional information about this process, and one of these is learning how to troubleshoot some errors.

Have contacted QBO chatline and they have progressed my query to a "senior product specialist" after going through all the steps you suggested to no avail.
