I'll share some information about the ACH form. The link for accessing the Authorization form in QuickBooks Online (QBO) is still not working.
Our engineers are currently investigating the source of the problem. Meanwhile, you can contact the QuickBooks Payment Team to inquire about alternative methods of downloading the ACH form.

Since you're still unable to access and complete the authorization form, I highly recommend reaching out to Merchant support team to further investigate the issue. They can also provide additional steps to help you resolve this.
In the meantime, you can look for an authorization form for ACH agreement from outside of QBO as a workaround.

This is why I hate the community. I have 10 extra web pages open all without the answer to the question. This link use to work but it does not now. The link now - wait for it - opens another community page!!! Community needs to be updated by the same people who make the changes in QBO. Links do not work.

Go to your MERCHANT CENTER merchantcenter.intuit.com
From the Home tab, click "Create a recurring payment"
THEN click on the link for the authorization form!

The link continues to not work when I write this on May 3, 2023. For such a large platform, it's really troubling that something like this hasn't been resolved in almost a year. I've never had a good customer service experience with QB and this is really frustrating. The platform doesn't provide a way for adding % fees (i.e. cc transaction fees) and doesn't provide a secure way to receive banking information. QB really needs to consider their customers' feedback and consider implementing these tools or risk losing a lot of their clientele.
This link doesn't take you to the form, just to the home page. How do I find the form? The link in QBO also just takes you to the same home screen. This really shouldn't be that difficult.

The link is already working. I logged in to the Merchant Services site and I was able to download the authorization form.

You're missing the entire point, what is the purpose of a link that doesn't work and has been an issue since Sept 2021?

Solving the problem would have been fixing it, I mean how hard could it be to attach a simple pdf to the link
100% always missing the point ..
This is a freaking simple html link to a pdf document. Two technologies that have been around for, I dunno, 30 years or more?
Yet they need an 'engineer' to fix the problem and you, the customer, needs to put in more support cases with more clueless people.
It's a joke, I'm going back to using pen and paper .
Oh, and don't forget, price went up again!

Just letting you know that the 2 links on the entry form shown in the screen shot go to a general support page now. I just switched to QBO and had to go into the desktop system to get the script it asked you to read to the customer. Please update.
