You don't have to manually fill out the 8655 form instead, you can directly enroll in electronic services. It'll automatically fill in the information for you. Let me walk you through the process.
Once authorization forms have been received and bank info has been verified, we will process the enrollment. You'll receive an email as soon as your payroll account is active for E-file & E-pay.

To know what information should be added in preparing the Form 8655, I suggest going to this IRS (Internal Revenue Service) website: https://www.irs.gov/pub/irs-pdf/f8655.pdf.

Let me further clarify my question: On the 8655 form I want to name QB as my agent. For that I need QB's EIN number and address to fill out the form. What is the appropriate EIN number and address for QB's as an authorized agent? Thank you

Let me share information about your electronic services enrollment depending on your payroll system.
To start with, the steps shared by my colleague above is the enrollment when you're using the enhanced payroll system. Since did see the E-file and E-pay settings, your payroll account may be one of the new systems we have.
With this, you'll see a different name to turn this feature on. Let me show you how:

This does not seem to work for Quickbooks Desktop Assisted Payroll. I cannot find the form on my in the QB App or in my account online. Please help.

Thank you for reaching out to the Community. I highly suggest contacting our Payroll Support team. One of our agents will be able to pull up your account to check why you are unable to locate the form.
Our Customer Care team's support hours are currently 6 AM (PT) to 6 PM (PT), Monday through Friday.
