I'll share some insights about setting up a QuickBooks account for International and US as a flexible account, @contactus4.
Please know that this ability isn't possible in QuickBooks Online (QBO). Guam is an unincorporated territory of the United States and has its own tax system, while Alaska is a state within the US and follows the tax laws of its federal government. As a workaround, you can set up a custom tax rate for the type of tax you need to track.
I can see the importance of setting up a QBO account to handle both international and U.S. transactions. With this, I recommend sending feedback directly to our product developers. They may consider adding this option to future updates to make our program better and more efficient for you to use. Please refer to the steps below:

I am finding it impossible to create the EFw2 by running the e-file state w-2 as instructed on the web. You cannot choose GU for a state and there are no territories listed. I have to submit the W2GU to Guam Rev and Tax and have to way to create the correct file format. Any suggestions?

Currently, QuickBooks Desktop (QBDT) doesn't support the electronic filing of State W-2s and 1099s. You can print W-2 in PDF format and manually enter the information Guam agency.

I understand that you're having difficulties creating your EFw2 file for Guam using the e-file state W-2 option. Let me guide you in the right direction on how to submit your tax forms.
As previously mentioned by my peer, the option to electronically file the W-2s and 1099s is currently unavailable in Guam. On the other hand, you'll have to visit the state website to check if they can provide other options to electronically file your tax forms.
