I need to know where the direct deposit reversal form is in QB desktop.

The direct deposit reversal form is not found in the QuickBooks Desktop system itself. You can start the application by following the steps below.
Please note that as mentioned above, reversal requests are not a guarantee that the funds will be recovered. Then, the reversal transaction takes 5 business days to process from the requested date. You can make a request between the 3rd and 5th day after the pay date.

Is the payment fee of $75 for the total process?
Let say I have 20 employees that we need to get the money back due to wrong payment. The total money to get back is $525 (19 employees for $25 each and 1 employee for $50).
What will be the reversal fee?

Yes, the reversal fee is $75 per payroll date regardless of the number of employees. Also, whether or not the reversal is successful, you'll be charged the said amount.

I tried to answer the form but it only allow me to add 10 employees. Can you teach me how to add more. I can't see an option or any box to click..

Thanks for trying. For this case, I would recommend reaching out to our Payroll Support team. That way, they can check for options for your direct deposit reversal request. They are available on weekdays from 6 AM to 6 PM PST.
