My W2s are preprinted, perforated, 4-up per employee. I do not see that option when I go to print them. The option for 4-up is there if I'm printing on blank paper, but not with forms. Am I missing something? Is there a preference or option setting elsewhere?

Yes, it's not available. Generally the best solution is to not print to pre-printed forms. There's no reason to and it's just harder because of alignment issues.

Before you print your W-2s preprinted forms, as an initial step, update your QuickBooks Desktop to its latest release and download the latest tax table to synchronize the changes.
Once done, go back to your W-2s and make sure to perform a print test in blank paper to check if the alignment is correct with the boxes in a preprinted form.

They only print properly on BLANK preforated paper for the 4up.
Not on the preprinted forms.
