Start by logging into your bank's site in a private window to confirm your sign-in info is correct, if you can't log in there, you'll need to reset your username or password on their end first. While you're on their site, check for any third-party access settings to ensure you've granted permission for QBO to connect. Once you've verified everything on the bank's side, head back into QBO and manually type your credentials into the edit sign in info section to avoid any auto-fill errors.
